A Simple Example: MOA Template
In a typical clinic workflow, your staff (MOAs) see patients before you. You want them to capture relevant clinical information, but not move into assessment or decision-making.
A simple template includes subjective information and vitals, while excluding assessment and plan—so the information you receive is structured and ready to use when you walk into the room. This can be created by adapting an existing SOAP note and removing unnecessary sections.

How to Create the Template
You can build this from an existing template—there’s no need to start from scratch.
Start by duplicating a template (such as a SOAP note), then modify it so it only includes the sections your team should complete. Adjust the formatting to match your EMR, including vitals or measurements.

Sharing and Using the Template
Once the template is ready, click Share, add your team members’ email addresses, and confirm.

They’ll receive a notification and can access it from the “Shared with you” section.

From there, they can import it into their templates and start using it right away. The original creator is shown for reference, and the template can be adjusted as needed after import.

Customizing the Template
Once imported, the template can be adjusted as needed while keeping documentation consistent across the clinic. This lets each team member work in a way that suits them, while keeping things consistent across the team.
